The Gunnison Valley Hospital Diabetic Self Management Education Program is a Federally certified diabetic education program through the American Association of Diabetes Educators.  Our program currently has two diabetic educators that have specialized training in diabetes.  Angie Merchant, RN is a Certified Diabetic Educator with over 3 years experience in diabetic education. Raelyn Christensen, RN is currently the Diabetic Coordinator for our program. Both instructors provide individual diabetic education and group education sessions as needed.

Diabetes is a disease that affects all areas of patient’s lives. Our diabetic education program strives to make it easier to control your diabetes, and to prevent the complications that diabetes can bring. Many times people rely on the opinions of others for their diabetic education.  The problem with this is that many times the information is incorrect or outdated. Through a Certified Diabetes Education Program, you will be taught the most current and correct information possible.

Most insurance companies will cover diabetic education, including Medicare and Medicaid. If you are unsure if your insurance company will cover the education, just give us a call and we will check your benefits for you.

Services offered by our program include:

  • Continuous Glucose Monitoring – This is a simple test that can record your blood sugars every 5 minutes for 3-5 days.  The insights provided by the CGM help your healthcare provider fine tune your diabetes treatment.
     
  • Individual Diabetic Education – Individual education sessions are available and include assessment, instruction, re-assessment, goal setting, and follow up. Let our educators work with you to make a personalized plan to improve your glucose control
     
  • Group Diabetic Education – Group classes are available as a way to decrease the cost of education, and are required for some insurance companies. Through group classes, we will still take the time to assess your personal situation, give you the instruction you need, and help you to make behavioral change goals.  This is a cost effective and effective alternative to traditional one- on- one education.
     
  • FREE Monthly Diabetic Support Group Meetings – Support group meetings cover many various topics related to diabetes including Proper Nutrition, Proper Exercise, Complications of Diabetes (and Prevention), Diabetic Medications, Pathophysiology of Diabetes, Healthy Coping, Sick Day management, What to do about Hyperglycemia or Hypoglycemia, and all the latest news and information related to diabetes.  As part if Support Groups, we also offer reduced cost A1C testing every 6 months (January and June).
     
  • FREE Monthly Newsletters – A diabetic newsletter is sent out to all patients associated with the diabetic program each month.  The newsletter provides excellent information for all diabetic patients.  Anyone interested in receiving the newsletter can call (435) 528-3955 or view the newsletter on-line at www.gvhomecare.org.
     
  • Annual Diabetic Health Fair – The annual diabetic health fair is held every June.  At the health fair, labs can be drawn at a reduced cost, multiple vendors are available, multiple free group classes are available, and a lot of FREE educational material is available.
